What Is a Kenalog Injection for Cystic Acne?

A Kenalog injection is a small, targeted cortisone shot placed directly into a deep cystic acne lesion. The medication, triamcinolone acetonide, is a corticosteroid that reduces inflammation, helping flatten painful cysts within 24 to 48 hours. It is considered an acute treatment for individual breakouts, not a long-term acne solution.
If you have dealt with deep, painful cystic breakouts, you already know that no amount of spot treatment is going to reach them. Cystic acne lies beneath the skin's surface and is driven by inflammation that topical products cannot calm. For the right lesion, a carefully administered Kenalog injection provides meaningful relief when you need it most.
Who Is a Good Candidate?
Kenalog injections work well for people dealing with:
Deep, painful cystic acne lesions
Breakouts that have not responded to topical treatments
Cysts that have been present for several weeks without resolving
Breakouts in a time-sensitive situation where rapid improvement is needed
Kenalog is not designed for whiteheads, blackheads, or mild inflammatory acne. It is reserved specifically for deep, inflamed cysts. Determining whether a lesion is appropriate for injection requires an in-person evaluation by a qualified medical provider.
What Happens During the Appointment?
The procedure is quick, straightforward, and much less intimidating than it sounds.
Cleansing: Your provider will clean the treatment area with an antiseptic solution to reduce infection risk.
The injection: Using an ultra-fine needle, your provider injects a small amount of diluted triamcinolone directly into the center of the cyst. The concentration matters here. Too strong a solution increases the risk of side effects, which is why dilution and technique are critical.
Duration: The actual injection takes seconds. Most clients describe it as a quick pinch or mild pressure, similar to a mosquito bite.
The appointment typically lasts 15 minutes or less. Afterward, you return to your normal activities right away.
What to Expect After Treatment
Results are not always instant, though many clients see meaningful improvement within the first 24 hours. Here is what is normal:
First 24 to 48 hours: The cyst begins to flatten, and the redness starts to calm. The pain or tenderness usually eases quickly.
Days 3 to 7: Most visible swelling has resolved for most clients. The skin might still look a little pink or feel slightly firm in the area.
Week 2 and beyond: The cyst should be significantly reduced. If residual inflammation remains, a follow-up injection is sometimes considered.
What Are the Potential Side Effects?
Kenalog is generally safe when administered by an experienced provider using the correct dilution. That said, there are a few side effects worth knowing about.
Skin atrophy (indentation): The most common concern with Kenalog is a small depression in the skin where the cyst was. This happens when the steroid temporarily dissolves some of the surrounding fat or collagen. Experienced providers minimize this risk by carefully diluting the solution and using a precise technique. If it does occur, it typically resolves on its own over several months as the skin regenerates collagen.
Hypopigmentation: The steroid sometimes lightens the skin at the injection site, leaving a lighter patch. This is more common in deeper skin tones and is usually temporary, though it takes time to resolve.
Telangiectasia: In rare cases, small visible capillaries appear at the injection site.
Systemic absorption: Because the dose used for acne is small, systemic side effects (the kind associated with oral steroids) are extremely uncommon.
Aftercare Tips for a Smooth Recovery
Keep the area clean and avoid touching or picking at it
Apply a gentle, fragrance-free moisturizer to support healing
Use SPF daily, especially over the treated area, to protect against hyperpigmentation
Avoid harsh actives like retinol or strong exfoliants on the area immediately after treatment
